Thank you for referring a client to NuPath Behavioral Health. To ensure we can review your referral quickly and accurately,
please complete the referral form and upload any supporting documents.

Eligibility Criteria for Adult Mental Health Services

  • Are 18 years or older
  • Have a mental health diagnosis verified by a licensed professional
  • Are enrolled in or eligible for North Dakota Medicaid
  • Live in North Dakota
  • Demonstrate a need for rehabilitative and/or therapeutic services to maintain or improve daily functioning and well-being. 

Referrals can come from physicians, social workers, hospitals, or community agencies. Individuals and families may also contact us directly to make a referral.

Eligibility Criteria for Children’s Behavioral Health Services

  • Are 6-17 years old (Should we put 10 and up?)
  • Have a mental health diagnosis verified by a licensed professional
  • Are enrolled in or eligible for North Dakota Medicaid
  • Live in North Dakota
  • Demonstrate a need for behavioral and/or therapeutic services to maintain or improve daily functioning and well-being.
