Community Based Services

At NuPath Behavioral Health, we believe that recovery and growth happen best in familiar environments — at home, within families, and in the community.

Our Home and Community-Based Services (HCBS), offered under North Dakota’s 1915(i) Behavioral Health program, are designed to help individuals with mental health and behavioral health needs live safely, confidently, and independently in their own communities. 

 

Care Coordination

Our Care Coordination services operate under a case management model designed to support individuals in navigating behavioral health, community resources, and daily living needs. We work directly with each person—and those they choose to involve—to ensure they receive the right services, at the right time, with the right level of support.

What Care Coordination Includes

  • Comprehensive assessment of needs, strengths, and goals
  • Development and ongoing review of an individualized care plan
  • Connecting individuals to behavioral health, medical, social, and community services
  • Coordinating communication between providers, supports, and agencies
  • Monitoring progress, addressing barriers, and adjusting services as needed
  • Supporting individuals through transitions, crises, or changes in care
  • Ensuring services are not duplicated and align with eligibility requirements

Independence starts with the right skills.

Our Skills Training and Integration Services help individuals learn and practice essential skills that promote autonomy and confidence in everyday life, such as:

  • Communication and relationship-building
  • Emotional regulation and stress management
  • Time management and organization
  • Cooking, budgeting, and self-care routines
  • Problem-solving and decision-making

These services are individualized and occur in natural settings — at home, in the community, or wherever support is most meaningful.
